Dear Patients,
We hope you and your family are doing well. Our community has been through a lot over the past few months and we are looking forward to resuming our normal habits and routines. However, in light of these events, we want to let you know how we are handling this situation moving forward.
- ✔ If you have an appointment and have any symptoms of COVID-19, please call us to reschedule. We still ask that you provide at least 24 hours notice when possible.
- ✔ Click here for a simple questionnaire. If you answer yes to any of these questions, please call us to reschedule any upcoming appointments.
- ✔ We are closely monitoring and following guidelines provided by the CDC, ADA, and OSHA.
- ✔ We are requiring all visitors to wear a mask. We will also be screening patients by reading temperatures and oxygen levels. Children may come with a parent/guardian, however we're asking that patients come alone when possible.
- ✔ Our waiting areas have been adjusted with social distancing and sanitization in mind. Our magazines have been removed as they are difficult to clean and disinfect.
- ✔ Please call us when you arrive in the parking lot. In an effort to keep everything moving smoothly, we will make the necessary preparations and move you directly to a room. Please be sure to follow the signs to the new exit when your appointment is done.
- ✔ If you are a new patient then we ask that you print out your paperwork, fill it out at home, and bring it with you to your appointment. This will help speed up your appointment and help us keep things moving smoothly. Click here to download the paperwork
